April 11, 2024--The Centers for Medicare and Medicaid Services (CMS) announced that it is currently testing a new Risk Based Survey (RBS) approach to Skilled Nursing Facility surveys. This is the first glimpse into a modified survey process that CMS has been developing in response to limited resources for surveys.
The purpose of the modified survey would be to conserve resources, by allowing for focused, less intensive surveys that take less time and resources than the traditional standard survey in facilities that are considered low risk. This would allow for a shift in resources toRisk-Based Survey (RBS) approach to Skilled Nursing Facility surveys. This is the first glimpse into a modified survey process that CMS has been developing in response to limited survey resources facilities where there may be greater health and safety concerns.
CMS states that “By modifying some surveys based on compliance and quality history, we will be able to devote more time and resources to nursing homes with lower quality whose residents are at higher risk of harm.”
It’s important to note that the RBS would only be used for recertification surveys, not for the complaint survey process. Early indications are that the number of RBS would be limited per state, with up to 10% of nursing homes qualifying.
The low-risk modified survey would conserve resources by allowing for focused, less intensive surveys that take less time and resources than the traditional standard survey in facilities designation would be determined by compliance and quality, potentially based on:
CMS notes that if concerns arise during the abbreviated survey, it would likely be converted to a full survey.
This risk-based approach could result in facilities that are better performers and provide higher quality care and services having a more focused, less resource intensive recertification survey.
